Person-Centered Collective proudly provides services to consumers of California Regional Centers through the Self Determination Program (SDP). Our services are designed to support person-centered outcomes, promote independence, and align with each individual’s IPP, preferences, and goals.
Teen and Adult Enrichment Services
Person-Centered Collective specializes in providing individualized person-centered intervention to teens and adults diagnosed with a developmental disability. Services are designed to support meaningful engagement, independence, and quality of life and may include skill development, social connection supports, and friendship matching aligned with your loved one’s preferences, interests, strengths, and IPP outcomes.
Staff Training and Ongoing Support
If your loved one is supported at home by caregivers or support staff (such as IHSS, HCBS, Direct Support Professionals, or Personal Care/Assistants), PCC partners with families to provide individualized assessments, goal development, training, and ongoing staff guidance. We help support staff create days that are not only safe and structured but enriching, enjoyable, respectful and centered on what matters most to your loved one.
