Our Purpose:

Our Purpose
At Person-Centered Collective, we believe meaningful support starts with meaningful connection.
We provide relationship-centered consultation, community integration support, and person-centered services for neurodivergent individuals, families, and organizations. Our work is rooted in the belief that quality care should be ethical, sustainable, individualized, and built around the real lives of the people receiving support.
Through consultation, assessment, facilitated social connection, and systems-focused guidance, we help individuals and organizations create environments where people can grow, participate, connect, and thrive within their communities.
What We Do
Person-Centered Collective was created to bridge the gap between high-quality clinical expertise and meaningful, sustainable support in everyday life.
We provide person-centered consultation, community integration support, and relationship-focused services for neurodivergent individuals, families, and organizations. Our work is designed to support growth, connection, independence, and quality of life in ways that are individualized, practical, and sustainable.
Services may include:
- Community integration and relationship-building support
- Facilitated friendship and social matching services through The Connection Collective
- Behavioral, social, and adaptive consultation
- Parent and caregiver support
- Adult and transition-age consultation services
- SDP-aligned consultation and guidance
- Limited strategic consultation for organizations and developing providers
Our approach is collaborative, neurodiversity-affirming, and individualized. We prioritize authentic relationships, practical support strategies, and services that integrate naturally into everyday routines, community participation, and real-world experiences.
